Ghosted as hard as I could but Drug Mob Casino is only theoretically ghostly; the guard distracted twice triggers ghost and even with temporary light knockout you’re not able to avoid it.
As for Abandoned Complex, I can safely say that you’d need TAS timing to get closer to a quiet rating with any consistency or less equipment. There may be a better position or strategy for it and Steve’s House but I can safely say that’s for someone else to try.

Does startiling an enemy count as being detected?
Startling how?
If everyone is on alert (the music gets intense), then you’ve been spotted.
The enemy will actively search for you, they will more likely try to group together in two-man teams, and sweep the area. In case they know where you are but not willing to charge, they might wait for you outside around the area covering specific doors and windows.
If they are not on alert, but checking around outside their normal patrol routes, then they are startled (Suspicious Activity – such status can be triggered by noise, sight of blood, doors open or lights off; not sure if broken windows also trigger this). In case you threw a bottle/soda can or empty magazine, you will trigger a Suspicious Activity event (startled) and will count in the level end results affecting your ranking.
