Boiler rooms will almost always be there if you walk a certain amount away from spawn in a certain direction.
Coördinaten
Eerst, we need to know about coordinates, X and Y are horizontal coordinates and Z is a vertical coordinate.
Your coordinates can be shown by toggling the debug menu (druk “G”).
They are above the “% chance for duller to spawn this second.” thing.
Boiler Room Findings
Through sandbox mode I discovered that if you walk 14k units in the X direction, there will almost always be a boiler room, I refreshed the map 4 times and they were all at 14k units X, and then I tried it in normal mode and it was at 14k units X too.
All of them also had 2 boilers “touching” which made a long hall of empty rooms in the middle.

Boiler Room Layout
Boiler rooms spawn in level 5, they always have a door in the center that leads to level 6 (or death).
Boiler rooms are a 5×5 Tiled Square, the outer tiles are always empty, the tile in the center is empty with a door (leading to level 6) in the middle.
The tiles in between the door tile and the empty tiles are always made of thin mazes (similar to level 2), the door is made of metal and is usually seen with a wisp around it.
Insanities and Dullers (commonly) can spawn in level 5 so watch out in the mazes, also don’t use your flashlight continuously since deathmoths can spawn in level 5.
Traveling
Toggle the debug menu, now walk forward in the direction that makes the X coordinate go up (X+), try to walk as straight as you can only changing direction when you encounter a dead end and going back to the X+ direction, and watch your step as level fun trapdoors spawn in the middle of halls.
